  • Monday – Saturday: 10:00 AM – 6:00 PM

4. Appointment & Scheduling Policy

  • All services require advance appointments. Walk-ins may be accommodated based on artist availability.

  • Clients must provide valid government-issued photo ID at the time of service.

  • Clients under 18 must have a parent or legal guardian present with valid ID.

5. Deposits, Payments, & Cancellations

  • Deposits: A non-refundable deposit is required to secure an appointment and will be applied toward the total cost of the service.

  • Cancellations: Must be made at least 48 hours in advance. Late cancellations or no-shows result in forfeiture of the deposit.

  • Payments: Full payment is due upon completion of services. Accepted methods include cash, credit/debit card, or approved digital payment options.

6. Client Eligibility & Consent

  • Services are restricted to clients aged 18 and over. Exceptions require written consent from a parent or legal guardian.

  • Clients must disclose any medical conditions, allergies, or medications that could affect the procedure.

  • By signing the consent form, the client acknowledges understanding the procedure, risks, and aftercare instructions.

7. Health, Safety, & Sanitation Policy

  • Lost Lotus Ink LLC complies with Utah Department of Health regulations for tattoo and piercing establishments.

  • All reusable equipment is sterilized using state-approved methods. Single-use items are disposed of immediately after use.

  • Technicians wear gloves and other protective equipment during procedures.

  • The studio maintains a clean, sanitary environment at all times, including regular disinfecting of surfaces and tools.

  • Clients with contagious illnesses, open wounds, or skin conditions may be refused service for safety reasons.

8. Tattoo & Piercing Procedure Policy

  • All procedures will be performed using sterilized, single-use needles and equipment.

  • Artists will discuss design, placement, and procedure expectations with the client prior to starting.

  • Clients may request modifications prior to or during the procedure, but the artist reserves the right to refuse designs deemed unsafe or unprofessional.

9. Aftercare & Healing Instructions

  • Clients will receive written aftercare instructions specific to their procedure.

  • Following these instructions is essential for proper healing and infection prevention.

  • Lost Lotus Ink LLC is not responsible for adverse outcomes caused by failure to follow aftercare directions.

10. Studio Conduct & Behavior Policy

  • Respectful behavior toward staff, artists, and other clients is mandatory.

  • Aggressive, abusive, or disruptive behavior may result in termination of service without refund.

  • Lost Lotus Ink LLC reserves the right to refuse service at management discretion in compliance with Utah law.

11. Privacy & Media Policy

  • Personal client information is kept confidential and used solely for studio business purposes.

  • Photos or videos of tattoos may be taken for promotional purposes only with explicit client consent.

  • Clients may request that their work not be photographed or shared publicly.

12. Liability & Waivers

  • Clients must sign a liability waiver before receiving services.

  • Lost Lotus Ink LLC is not responsible for:

    • Allergic reactions or medical complications

    • Injuries due to failure to follow aftercare instructions

    • Dissatisfaction due to subjective artistic interpretation

  • By signing, clients accept all risks associated with tattooing or piercing procedures.

13. Employee & Staff Policies

  • Staff must adhere to all sanitation and safety standards.

  • Professional conduct toward clients and coworkers is required at all times.

  • Employees are prohibited from performing services under the influence of drugs or alcohol.

  • Staff must complete ongoing training and certification as required by state law.

14. Emergency & Incident Procedures

  • First aid kits are available on-site.

  • Any injuries or incidents must be reported immediately to management.

  • In the event of an emergency, local emergency services will be contacted, and clients will be assisted accordingly.

15. Amendments & Contact

  • Lost Lotus Ink LLC reserves the right to update or modify policies at any time.

  • Clients and staff will be notified of significant changes.
